Fastbreak Lacrosse Camp Registration
Online registration is now closed. You may still register onsite on Sunday morning. Please print and complete the registration form and bring it to registration on Sunday, July 20th. Additionally, please call (716) 636-1152 to inform that you will be registering onsite. Thank you for choosing Fastbreak Lacrosse Camp!
Refund Policy
Refunds will only be given in the event of injury or illness. An injury or illness that results in a camper missing for the entire five day's of camp will require a note from your family doctor. Before June 1st, the amount equivalent to the half of the deposit is non-refundable ($75), On or after June 1st the amount equivalent to the entire deposit is non-refundable ($150). On or after July 16th, no refund will be given and/or 100% or the full price of camp will be kept unless the camper has a medical excuse from your family doctor due to injury or illness. Any camper dismissed from camp due to disciplinary reasons will not be granted a refund.
Liability Waiver
By registering for Fastbreak Lacrosse Camp, you are agreeing to the following:
During the week of camp my son will be involved in contact activities that are a part of the game of lacrosse. I assume all foregoing risk & accept personal responsibility for any damages following such injury, permanent disability or death. I am also hereby waiving and releasing the Fast Break Lacrosse Camp, its Directors, all staff and Sweet Home Central School District from any and all liability for injuries incurred by my child while attending camp.
Fastbreak Individual & Camp Photo
At the Fastbreak Lacrosse Camp we offer our campers the opportunity to have an individual & camp photo for a future lacrosse momento. You can have an individual photo as well as receive the camp photo with various reasonable packages. Click the link below and print the registration form.
Do not mail in this form, please print this form and bring it to registration on Sunday, July 23rd with the exact amount of money enclosed. Hermann Studio's will be at camp during registration (on July 23rd) to answer all of your questions and provide you with any information that you may need.
The individual photo's will be taken on Monday morning (July 24th) between 8:15AM - 9:00AM. Team photo will happen at 9AM.
